Indications:

This Amino acid is a specialized parenteral nutrition solution used primarily in patients with liver dysfunction and those requiring nutritional support through intravenous administration.

Approved & Common Indications
🌿 Liver Disease

  • Liver Cirrhosis and Hepatic Failure: For nutritional support in patients with liver cirrhosis or hepatic failure, particularly in cases where protein metabolism is impaired.
  • Hepatic Encephalopathy and other forms of liver dysfunction.
  • Liver Regeneration and support in critical care settings.

🌿 Parenteral Nutrition

  • For patients unable to receive adequate nutrition via oral or enteral feeding, including:
    • Critically ill patients
    • Post-surgical patients
    • Patients with gastrointestinal conditions that prevent nutrient absorption

🌿 Protein Deficiency & Catabolic States

  • For patients with:
    • Significant protein loss or malnutrition due to medical conditions (e.g., trauma, burns)
    • Elevated protein and amino acid needs due to catabolic states, where absorption is impaired

Dosage & Administration:

Recommended Dosage
🌿 Adults with Hepatic Insufficiency

  • Individualized based on protein requirement, hepatic function, and body weight.
  • Usual recommended dose:
    • 1.0 to 1.5 g amino acids/kg body weight/day
    • For a 70 kg patient: 700 ml to 1050 ml/day

Route of Administration
🌿 Intravenous Infusion (IV)

  • Administer via central venous catheter due to high osmolarity.
  • Should be combined with adequate calories (glucose/lipids), electrolytes, vitamins, and trace elements as part of total parenteral nutrition (TPN).

Infusion Rate
🌿 Maximum Rate:

  • Do not exceed 1.25 ml/kg body weight/hour
    • For a 70 kg adult: up to 87.5 ml/hour
Mechanism of Action (MOA):

Aminosteril N-Hepa provides a blend of essential and non-essential amino acids that are crucial for protein synthesis and tissue repair, particularly in patients with liver disease. These amino acids help improve nitrogen balance, support liver function, and aid in tissue regeneration, which is vital for patients with compromised liver function. The infusion helps restore protein synthesis capabilities and maintain overall metabolic health during recovery.

Pregnancy Category & Lactation:
  • Pregnancy Category: C – Potential risks to the fetus are unknown. Use only if the benefits outweigh the risks.
  • Lactation: Caution is advised. Limited data is available, so consult a healthcare provider before use during breastfeeding.
Contraindications:
  • Hypersensitivity to any of the components in the solution.
  • Severe renal failure without dialysis (due to the risk of amino acid accumulation).
  • Severe metabolic acidosis or uncontrolled hyperkalemia.
  • Severe dehydration or fluid overload.
Side Effects
  • Common: Hyperammonemia, fluid imbalance, infection risk, gastrointestinal symptoms (nausea/vomiting).
  • Serious: Allergic reactions (rash, itching), severe electrolyte disturbances.
Drug Interactions:
  • Renal-affecting drugs: Caution with diuretics, as they may cause electrolyte imbalances.
  • Potassium-sparing drugs: Risk of worsening hyperkalemia.
  • Insulin/glucose: May affect insulin requirements, especially in diabetic patients.
