Approved Indications

• Treatment of Herpes Zoster (Shingles) in adults caused by Varicella-Zoster Virus (VZV).

Off-Label / Clinically Accepted Uses

• Limited clinical and investigational use in Herpes Simplex Virus (HSV-1 and HSV-2) infections.

Adults

• 400 mg orally once daily after meals for 7 days
• Treatment should be started as early as possible, preferably within 72 hours of rash onset.

Pediatrics

• Safety and efficacy have not been established in pediatric patients.

Elderly

• No specific dosage adjustment generally required.
• Clinical monitoring is recommended.

Special Populations

• Renal impairment: Use with caution in severe renal dysfunction.
• Hepatic impairment: Use cautiously in moderate to severe hepatic impairment.

• Take after meals.

Amenamevir inhibits the viral helicase-primase complex, an enzyme essential for herpesvirus DNA replication. This action suppresses viral multiplication and helps reduce the severity and duration of herpes zoster infection.

• Absorption: Rapidly absorbed after oral administration.
• Distribution: Highly protein bound (>95%).
• Metabolism: Primarily metabolized in the liver via CYP3A enzymes.
• Excretion: Mainly eliminated through feces; minor urinary excretion.
• Half-life: Approximately 7–8 hours.

Pregnancy

• Limited human safety data available.
• Use only if the potential benefit justifies the potential risk.

Lactation

• It is unknown whether amenamevir is excreted in human breast milk.
• Caution is advised during breastfeeding.

Antiviral Agent (Helicase-Primase Inhibitor)

• Known hypersensitivity to amenamevir or any component of the formulation.

• Use cautiously in patients with hepatic impairment.
• Concomitant use with strong CYP3A inducers or inhibitors may alter drug levels.
• Early initiation of treatment improves clinical outcome.

Amenamevir is approved in Japan for herpes zoster treatment and is recognized as a novel antiviral with a unique helicase-primase inhibition mechanism.
